The form that is free to download below, can help EMS treat you faster in an emergency. Please fill out the form and print out. Make several copies for yourself and give one to the EMS workers in the event of an emergency. Time saved....is a life saved. |
There are some misconceptions out in the public, that if a patients wish is to not be resuscitated, that all they
have to give the EMS workers if an ambulance is called, is their living will. In the state of Kentucky, if your
wish is to not be resuscitated by the ambulance service, you must have an EMS DNR. The EMS DNR is
only for the ambulance service. If this form is not filled out, is not the original and not present at the time
EMS arrives at your home, we must do everything in our power to resuscitate you. There are many things in
life that may happen to someone....terminal cancer, old age with many health complications, etc. Due to
these complications, it may be the patients wish that if their heart should stop beating and if their breathing
should cease, that they do not want EMS to try to restart their heart, or place a tube in their lungs to try to get
them breathing again. We respectfully want to have the up most consideration for a patients dieing wish, but
without this EMS DNR, we would have to do everything in our power to resuscitate you. Please click on the

your wish, please post it somewhere in your home that is easily accessible. Such as above your bed or on
the refrigerator. On this form, there is even a place to fill and cut out to place in a bracelet that could be
